• Free Standard modem with Qwest Connect Platinum, except for Standalone(normally $59.99)
• $40.00 for Advanced Modem with Qwest Connect Platinum or $69.99 for Standalone Platinum (normally $99.99)
• $30 monthly recurring charge for Qwest Connect Silver Standalone service with Price for Life* (normally $33 per month)

• Available for new and existing residential subscribers
• All DSL plans are eligible except 256 Kbps
• Subscribers ordering Price for Life must agree to a 2-year term commitment. Subscribers will continue to receive the term pricing after successful completion of their 2-year term. Subscribers are assessed an early termination charge ($200) if they break the commitment.
• Subscribers can keep Price for Life if they:
  • Move to a new location within current state
  • Change phone numbers
  • Change DSL speed to anything other than 256kbps
  • Change from wireline to standalone or vice versa
• Subscribers will lose Price for Life if they:
  • Disconnect the Qwest DSL service
  • Change DSL speed to 256 kbps
  • Remove the term commitment
  • Do a change of responsibility on the account
  • Change from an Independent ISP to Qwest.net Basic or MSN Premium
  • Change from Qwest.net Basic or MSN Premium to an Independent ISP

**All DSL speeds are eligible with a 2 or 3 year term commitment for business subscribers. Subscribers will continue to receive the term pricing after successful completion of their 2 or 3 year term. Subscribers are assessed an early termination charge ($200 for 2 year; $300 for 3 year) if they break the commitment.

Customers who ordered a DSL modem before July 31, 2004 or who were renting modems before that date will be billed the prior $3 monthly rate and will not be subject to the modem rental rate increase. The modem purchase rate of $59.99 did not change.

Customers who change speeds, or otherwise make changes to their previously existing Qwest DSL service, will continue to be billed their same modem rental rate -- such changes will not affect the modem rental rate.

Actual DSL speed varies depending on service location, phone line qualifications, computer performance/configuration, network/Internet congestion and other factors. Uninterrupted or error-free service not guaranteed. Computer requirements: Windows(r) 98 or later; further WiFi equipment may be required to enable additional computers. A customer who accepts a promotional rate for a rental modem and terminates his service prior to the expiration date of the promotional period, is assessed:

• A restocking fee of $25, PLUS
• Unbilled modem rental charges at the full monthly rate of the modem for every month the customer has had the modem

Modem rental charges continue until the modem is returned.
